Safe Children Vietnam: Findings from a training intervention to improve the recognition and response to child abuse and neglect in the ED

Abstract

Aim

With appropriate training, tools, and institutional policy, nurses and other health professionals can effectively recognise and respond to children with abuse-related injuries. Despite the comparatively high incidence of child abuse and neglect (CAN) in Vietnam, there is no formal training, or consistent hospital policies and protocols for recognition and response to these cases. This study evaluated the effectiveness of the Safe Children Vietnam Train the Trainer program  for improving emergency nurses’ knowledge and professional self-efficacy concerning CAN response.

Method

Participants were recruited from a leading paediatric hospital in the south of Vietnam. Nurses and medical staff from the hospital’s ED, Outpatient Department (OPD) and Burns Unit participated in the training intervention and follow-up in 2013/2014. Quantitative data were collected from 127 participants at three time-points: immediately prior, 6-weeks after and again 6-months after the training intervention. The multi-modal program consisted of a workbook, two-days of interactive workshops, DVDs, posters and inservice presentations. All training materials were translated to Vietnamese language .

Results

At baseline, only 3.9% (n=5) of the sample had previously received formal training related to CAN. Improvements were observed in participant knowledge, intended reporting behaviour, and professional self-efficacy. Improvements were not sustained at 6 month follow-up indicating the need for ongoing training.

Conclusions

This is the first study of its kind in Vietnam. Regression analyses support the effectiveness of the Safe Children Vietnam training intervention in improving clinician knowledge and professional self-efficacy in recognising and responding to CAN presentations in the hospital setting, with implications for widespread implementation. Results at long-term follow-up reinforce the need for ongoing training to ensure these improvements are sustained in the long-term.
